He or she may do so in the absence of a temporal leader, as in Tepest, as both a spiritual and a temporal leader, as in G'Henna or Nidala, or with the backing of a temporal leader who in turn is backed, as in Borca or Nova Vaasa.
The leader of a state religion such as the Church of the Lawgiver or the Church of Zhakata is a clear example. The leader of the Cult of the Wolf God or the Church of Belenus would also meet the criteria. The leader of the Cult of the Ancestral Choir or the Cult of the Morninglord would probably not wield sufficient influence.
