Exists on the Prime Material Plane or, like the Demiplane of Dread, in the Etheral Plane?

Deliberately undefined in the 2e boxed set. (It just says there may be a link to Ravenloft.) The 3e book makes a definitive link, saying that the Red Death came from Ravenloft (but stopping just shy of confirming that it was one of the Dark Powers), but says that the world is cut off from other planes, making planar travel impossible.
