Also, if there's no place close enough to the house for Raen to immediately be aware of combat inside or it seems like Kat moved far enough, he will enter the house.

Yes, I have interpreted the posts as Sara patrolling, circling, and keeping lookout.

Raen had been watching the door. Then he went inside and deliberately turned his back on the open door, as per player's post.

The person who closed the door somehow escaped detection long enough to enter the alley and close the door. Perfect timing. Impressive stealth and quickness.

What did Kat save against?

Assuming that the answer is not too dramatic she will seek for trouble, in full Combat Expertise mode, until she finds it and which point its over to the feint/attack combo.
