Appropriately named for the throngs of ghasts and ghouls they command, Ghoul Lords are much more powerful and intelligent forms of the undead ghouls and unique to the Demiplane of Dread. Rumor attributes the origins of ghoul lords to a different source from their lesser kin. Instead of ghoul lords having been cannibals in life, the rumor has it they were created by a single incredibly evil necromancer, who is also rumored to be the current darklord of the Nightmare Lands.[1]
As such, there may be a limited number of them.
Ghoul lords are known to be found in the following domains: Barovia[2], Falkovnia[3], Hazlan[4], Necropolis[5], Nova Vaasa[6]
As a type of undead, they can be found in Darkon[7][8], Har'Akir[9][10], Mordent[11][12], Sanguinia[13], Sithicus[14][15], and Valachan[16][17] Ghoul lords regulardly command entire packs of ghouls in the sewers beneath Citadel Cavitius.[18]

“I am the lady of all I survey”, she stated with manifest glee as her barely clothed arm, with that leathery skin and taloned hand, cut a grandiose arc over the vast graveyard, leaving greenish wisps of vapour in its wake. She continued, “A small kingdom it is, but it is mine and we do eat good, very good.” Then she turned her ill eye upon me, “Your slave is sick and fearful in my presence, but you not. I know why. You share the hunger. You would sup with me. What would you have my slaves fetch us?”
