For reasons only known to Nerull, an avatar of Nerull was assisting cultists of the Eternal Order with gathering souls for the events of Death Unchained. For equally mysterious reasons, the Avatar may brought a group of adventurers into Ravenloft to interfere with the process.[1] 
Note that this Avatar of Nerull was never updated to 3rd Edition, so the creature types below are speculative.
