If I'm being super-generous, I could say that with the core dissolved and Richemulot alone in the Mists, rats might be the only major wildlife available, so making them into luxury furs could make a little sense. I mean, I find it weird and off-putting to see mink heads dangling off a stole, so this isn't that much weirder. But yeah, this looks like the artist tried really hard to deliver on an absurd art brief, and there's just no saving it.

It's not Alanik's fall, injury, or him being bound to a wheelchair that rubs me the wrong way. It's his choice to remain in the wheelchair. It's illogical, given that Wizard's standard (print) version of Dungeons and Dragons has plenty of ways to heal such an injury. So, assuming they continue with a similar level of magic, then we are expected to accept that he chooses the wheelchair over healing. That's a sell that reeks.
Of course, if they (or we) accept a lower level of magic, then it's a non-issue. A cool tweak to a fairly cool NPC's history.

Heal, Regeneration, Greater Restoration. You could make an argument for Healing Word or Cure Wounds too, and they’ve level 1 spells

True Resurrection is the only spell that lists replacing injured or missing internal organs among its effects. Wish could arguably do the same thing, while Regenerate, though technically only restoring missing limbs, might be stretched to permit it by a sufficiently generous DM.

Any spell that you use to keep your fighter's intestines in his body really. But specifically, Lesser Restoration (2nd level spell) should do the trick, unless you rule the paralyzed condition a mental or temp one only, in which case any of those Drinnik listed. Heal, or Regenerate if you want to highlight seriousness of injury...
And if you elevate spinal cord injuries to a level beyond magic then you should probably rethink your healing magic in general. Wouldn't make sense to heal gaping wounds, limbs etc and not spinal cords...but that's just me.

A DM can absolutely rule that those spells would be sufficient in their game. By the rules through, hit point loss never results in permanent injuries, so effects that restore the former won't necessarily cure the latter. Again, unless the DM decides to allow it. That would also probably mean that the same spells would restore Gennifer Weathermay-Foxgrove's damaged eye and similar afflictions.
EDIT: As for Lesser Restoration, I don't think it would work because Alanik is not under effect of the Paralyzed condition. Unless a ghast attacked him, or some such.

We should probably acknowledge that D&D healing is bonkers in general. It makes no logical sense that a Potion of Healing that completely restored a near-dead commoner would barely heal a scratch on a high-level fighter. But here we are.

The paralyzed effect of a spinal injury feels like physical effect not healed by
lesser restoration. Like you wouldn't regrow eyes that were removed just because it removes blindness.

Seriously, if you want to limit the healing like that (high, almost max NPC/setting level only), then that's the only saving grace for keeping Alanik in a wheelchair. Even then, given his abilities and no doubt favours owed by a multitude of past and current clients, it's only a matter of time until he'll go for the heal. In my head canon that is.
For the record, I always ran Ravenloft low magic. So Alanik would probably never get to walk again, given an injury like that.
I mention it because it stands out, as presented. It's still illogical to me. Though I'll admit it can work, if the stars are aligned...

It's interesting that Madam Eva (according to Curse of Strahd) has it in her spell list.
Also Viktra Mordenheim is described as being able to heal or replace any body part, so she's another option
